docs: link semantic indexing guide from hub, index, and AGENTS.md #1

The semantic-indexing.md guide existed but was undiscoverable. Add
references from docs/README.md, docs/INDEX.md, root AGENTS.md, and the
vtcode-indexer module doc so the opt-in vector/semantic search feature is
reachable from the main entry points.
